NDMS Study Hall Permission Form

Students staying after school for a game must have a permission form signed and $4 or ALL SPORTS PASS. Permission forms may only be turned in the morning of the game to your child's homeroom teacher.

 

Forms are available in the Gym Lobby and inside the Girls Locker Room. If a student forgets to pick one up you may print one at home.
